供应商信息披露 Statement

The State of Alabama requires that a 供应商信息披露 Statement be completed for all proposals, bids, contracts, or grant proposals in excess of $5,000. Once completed by the vendor or subcontracted, the Disclosure Statement must be filed with various State Agencies. The 采购部门 will coordinate the distribution of these completed statements to the appropriate state agencies.

Payment to vendors cannot be made without a completed 供应商信息披露 Statement.
